Every message is read and sorted into one of eight kinds: reschedule, new client, clinical question, billing, refill, safety, sales — and not sure. Five of those arrive with the reply already drafted, waiting for you. When a message could be two things, it goes to not sure and waits for you to decide.

Pablo works in the inbox you already have. Connect your practice Gmail and he works right there: same address, same website, same directory listing, same business cards. If you'd rather start clean, a HIPAA-eligible mailbox comes with your plan. Our BAA is signed the day you join, and we'll walk you through getting your existing mailbox covered in the same week. His replies land as ordinary drafts in that mailbox — open it and they're sitting there, so you can edit or send from Gmail itself if you'd rather.

You control what Pablo can send

You decide how much Pablo can send without you: one setting per kind of message, not one switch for everything. For each of the eight kinds you pick one of three:

  • File it only.Pablo sorts it and stays out of your way.
  • Draft for review.Pablo writes the reply, you read it and send it. The default for everything.
  • Send it himself.Pablo replies without you.

Start with everything on draft-for-review. Move one category up when Pablo has earned it.

Some messages always require your review. Clinical questions, refills, safety flags, sales mail, and messages Pablo couldn't confidently place cannot be set to send it himself. A safety-flagged message goes straight to you without a draft.

And before any reply goes out on its own, it gets one more read: anything drifting toward medical or legal advice is held for you instead.

Open-source clinical records

Pablo's clinical record system is open source under the AGPL, and v1.0 shipped in July 2026. It holds sessions, notes, and client records and can be inspected or run on your own cloud. The inbox service is not open source.

BAA and mailbox included

Your Business Associate Agreement is executed when you sign up, with no forms to chase. A HIPAA-eligible mailbox is included in your price, whether you forward the address you already use or take a fresh one. Data is encrypted in transit and at rest, US-based, never used to train models, never sold.

Time to take your records with you

After cancellation, your records remain available to read and export for 60 days. That gives you time to download a complete copy before your practice data is securely deleted.

You approve every word

Drafts, not sends, unless you raise the dial yourself, and never for clinical mail, refills, or anything safety-flagged. Notes are never filed automatically. Your license, your judgment, every time.

Usage-based costs. Notes from uploaded or pasted transcripts are unlimited. Optional session recording includes 50 sessions a month, then costs 40¢ per session. Carrier charges for services such as SMS and future phone lines are passed through without markup.
