Pablo is an AI front office for a solo practice. He starts at the desk that costs you the most evenings: your practice inbox. He reads every message, sorts it, and has the routine replies drafted before you open it. The one flagged for safety he will not touch — no draft, no autopilot. It goes to the top of your queue with crisis resources pinned underneath, waiting for you.
The notes are one part of it. The rest is mail: the inquiry that goes cold, the reschedule that eats twenty minutes, the billing thread you reread three times. All of it lands after hours, because that's when you have hands free.
Every message is read and sorted into one of eight kinds: reschedule, new client, clinical question, billing, refill, safety, sales — and not sure. Five of those arrive with the reply already drafted, waiting for you. When a message could be two things, it goes to not sure and waits for you to decide.
Pablo works in the inbox you already have. Connect your practice Gmail and he works right there: same address, same website, same directory listing, same business cards. If you'd rather start clean, a HIPAA-eligible mailbox comes with your plan. Our BAA is signed the day you join, and we'll walk you through getting your existing mailbox covered in the same week. His replies land as ordinary drafts in that mailbox — open it and they're sitting there, so you can edit or send from Gmail itself if you'd rather.
You decide how much Pablo can send without you: one setting per kind of message, not one switch for everything. For each of the eight kinds you pick one of three:
Start with everything on draft-for-review. Move one category up when Pablo has earned it.
Some messages always require your review. Clinical questions, refills, safety flags, sales mail, and messages Pablo couldn't confidently place cannot be set to send it himself. A safety-flagged message goes straight to you without a draft.
And before any reply goes out on its own, it gets one more read: anything drifting toward medical or legal advice is held for you instead.
Practice email takes time whether you handle it yourself or pay someone else to do it.

Pablo can work alongside the EHR you already use for scheduling and billing. If Pablo already covers what your practice needs, its clinical record system is included at no extra cost.

Pablo spots new-client inquiries as they arrive and drafts a reply using the fees, availability, and insurance information you've provided. You can respond while the person is still looking for care.
Pablo reads the edits you make to his drafts and proposes changes to how he writes: your sign-off, your greeting, the phrases you keep deleting. Each change is a suggestion you accept or reject, one at a time.
The session ended at six and the note is still unwritten. Hand Pablo the transcript, by upload or paste, and the draft is waiting by the time you've packed up: SOAP by default, plus DAP, BIRP, GIRP, intake, safety plan and treatment plan. Pablo drafts. He never signs, and he never files.
In-session recording and transcription are available on request rather than switched on by default.
Pablo holds the chart already: documents, medication lists, the run of session history. Ask him a question about a client and the answer comes back drawn from your own notes, intake and treatment plan, with the documents he used shown beside it so you can check his work. When the chart does not support an answer, he says so instead of filling the gap.
Set your working hours once. When someone asks to move an appointment, Pablo reads your real calendar and offers up to three genuinely open times in the reply. What he can't do yet is take "Tuesday at four works" and put it in the book. You still close that last step.
The hours ledger, category tracking, and tamper-evident supervisor countersignature are built and tested. The interface is still in design and isn't available yet.

This feature is under review. Founding members will see it first, as part of their existing plan.
Pablo doesn't yet send or collect consent forms, insurance cards, and other intake paperwork. Building that requires a secure client-facing portal with identity checks and document follow-up.
Pablo doesn't answer calls or triage voicemail today. If live phone coverage is your main need, you'll still need an answering service. Voice support remains on the roadmap, with additional safeguards for callers who may be in crisis.
Why one feature at a time? Pablo works under a Business Associate Agreement signed when you join. Each new feature creates another path for client information, so it is designed, tested, and reviewed before release. Inbox access is rolling out one founding practice at a time while we monitor how it performs in real use.
Chief of Staff will bring together what needs your attention, what's slipping, and which clients to review first. It is under review, and founding members will get access first as part of their existing plan.

Pablo's clinical record system is open source under the AGPL, and v1.0 shipped in July 2026. It holds sessions, notes, and client records and can be inspected or run on your own cloud. The inbox service is not open source.
Your Business Associate Agreement is executed when you sign up, with no forms to chase. A HIPAA-eligible mailbox is included in your price, whether you forward the address you already use or take a fresh one. Data is encrypted in transit and at rest, US-based, never used to train models, never sold.
After cancellation, your records remain available to read and export for 60 days. That gives you time to download a complete copy before your practice data is securely deleted.
Drafts, not sends, unless you raise the dial yourself, and never for clinical mail, refills, or anything safety-flagged. Notes are never filed automatically. Your license, your judgment, every time.
